Professional Dilemmas is half your MSRA score, and most question banks just mark it right or wrong. The exam doesn't. Practise with marking that works the way the paper does, and see exactly which Good Medical Practice principles are costing you marks.
The MSRA has two papers. Clinical Problem Solving tests what you know. Professional Dilemmas tests what you would do: situational judgement pitched at the level of an F2 doctor, scored against three domains. Both papers count equally.
In a ranking item each of the four or five options earns marks by distance from its correct place. Swap two neighbours and you keep most of the marks. Put the worst option first and you lose them. Practising with a right-or-wrong bank teaches you nothing about which.
You are an F2 on a medical ward. Reviewing a drug chart you notice that a colleague who finished their shift two hours ago has prescribed a penicillin-based antibiotic for a patient with a clearly documented penicillin allergy. The dose has not been given but is due in 30 minutes.
Rank these from most (1) to least (5) appropriate:
Key: A, B, C, D, E. A candidate who answers A, C, B, E, D scores 16 of 20.
| Option | Key | You | Marks |
|---|---|---|---|
| A | 1 | 1 | 4 |
| B | 2 | 3 | 3 |
| C | 3 | 2 | 3 |
| D | 4 | 5 | 3 |
| E | 5 | 4 | 3 |
| Total | 16 / 20 | ||
Every option's explanation cites the Good Medical Practice paragraph behind it. After each attempt we add your marks lost to every paragraph the item tested, so your Review page shows the principles you keep getting wrong, not just a percentage.
One click starts a practice set of every scenario that tests the paragraph you are weakest on.
150 original scenarios, 50 in each domain, every option explained: 110 ranking and 40 choose-three, ranking-heavy by design because a ranking item carries 20 marks against 12.
The paper assesses three professional attributes. Every scenario here targets one, and your Review page breaks results down the same way.
The big MSRA banks are clinical banks with a few hundred PD items marked right or wrong and a one-line explanation of the best answer. That is not how the paper is marked, and it does not tell you what to fix.
